Why the clinic writes first

A patient flying in has one thing a local patient does not: a fixed number of days. A course that needs more of them than the trip allows is worth knowing about before the flight is booked.

So every enquiry is answered in writing by a dermatologist, and the answer sometimes says that the treatment is not worth the journey.
